MDA is pleased to offer on-line registration for the annual conference this year.  It's a quick, easy, and secure two-step Process.

Step 1: Simply fill out the entire form below with your Registration Information, Meal Selections, Breakout Session Selections, and Special Interest Selections.  If you would like to view the Conference Fees before selecting your sessions, scroll to the bottom of the page for a listing of fees.  Once you have completed the form, be sure to check it for accuracy, as you will not have a chance to change your selections once you click the "Submit" button. 

Step 2: Once you click the "Submit" button below, you will be directed to a confirmation page where you will complete your registration by making a secure payment using any major credit card or your PayPal Account.

Cancellations/Refunds:  Cancellations received in writing by April 14 will be refunded
less a $55 processing fee.  No refunds after April 14.


If you are not an ADA Member you can join now:
