MDA Checklist for Continuing Education Application Materials- Use this helpful checklist to be sure you are submitting everything you need to MDA for approval of your CPE program.
CDR form RC-3- Must complete this form to apply for CPE approval.
MDA Continung Professional Education Application Must complete this form to apply for CPE approval.
MDA Speaker Information Form- Use this form if you do not have a copy of the speaker’s curriculum vitae or resume.
Certificate of Completion- Program Providers can use this form to provide proof of attendance for program participants.
